Job Title: Wraparound Lead Care Coordinator

Wisconsin Community Services, Inc. is seeking a highly motivated and compassionate individual to fill the role of Wraparound Lead Care Coordinator. This position plays a critical role in providing support to Care Coordinators in all Wraparound Programs, while also providing individualized care coordination for children, adolescents, and young adults.

Key Responsibilities:
  • Provide leadership and role modeling of the Wraparound process for Care Coordinators.
  • Assist Supervisors with development of care coordinators, including mentoring, training, and coaching.
  • Provide hands-on training in the community with new Care Coordinators for the first six months of employment.
  • Attend Team/Plan of Care Meetings and Court Hearings to provide coaching for staff in the Wraparound Process, policies, and procedures.
  • Maintain a caseload of approximately 6-8 children and families.
  • Contact enrolled children and families within 48 hours of enrollment and set up initial visits.
  • Schedule and attend required face-to-face visits and facilitate Child and Family Team Meetings.
  • Develop and implement individualized Plans of Care based on identified strengths and needs.
  • Monitor the provision and quality of services provided to families and advocate for children and families across various settings.
  • Attend all scheduled court hearings as a representative of the designated Wraparound program.
Requirements:
  • Bachelor's degree in social work, human services, or a related field.
  • Minimum of one year of Wraparound care coordination experience.
  • Working knowledge of computer applications, including Windows and Microsoft Word.
  • Cultural competence and ability to respond respectfully and effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.
Working Conditions:

The work is performed primarily in a busy office environment managing caseloads of children, young adults, and families. Some of the work is done sitting at a desk using a computer, and requires outreach in the community and families' homes. Much of the outreach is done in urban neighborhoods, and several hours per day may be spent driving.
