Key Responsibilities

We are looking for a Full-Time Care Transition Nurse.

The Care Transition Nurse (CTN) plays a vital role in achieving the organization's goals as a field-based representative, consistently meeting referral and admission targets.

  • Facilitates patient referrals to the organization and collaborates with discharge coordinators to ensure high-quality home care services.
  • Assesses patient eligibility for infusion services.
  • Communicates effectively with relevant healthcare professionals involved in the care of potential home infusion patients, providing valuable insights and recommendations.
  • Cultivates professional relationships with healthcare providers and facilities to foster open communication and support infusion objectives.
  • Serves as a resource for discharge planners, physicians, and hospital staff to evaluate patient eligibility for infusion services.
  • Clarifies Medicare and Medicaid guidelines, insurance benefits, financing options, and organizational policies.
  • Upon patient selection of our services, coordinates with attending physicians to secure necessary orders for home care.
  • Assists in procuring equipment essential for delivering appropriate home infusion therapy.
  • Identifies individual patient needs for services, community resources, and training, providing instructions on equipment and procedures.
  • Participates in educating new providers and staff about home infusion and the significance of effective communication with clients.
  • Offers general infusion education to patients, caregivers, and others as requested.
  • Collaborates with the Director of Operations and other staff to ensure seamless patient care.
  • Engages with patients across various age groups, including neonates, pediatrics, adults, and geriatrics, ensuring comprehensive care.

Qualifications

  • Graduate of an accredited nursing program.
  • Experience in a Care Transition Nurse role or similar, with a background in sales or marketing.
  • Nursing experience in hospital or homecare settings.
  • Valid RN or LPN license in the state of practice.
  • Current CPR certification; capable of performing CPR.
  • Compliance with state certification/registration and organizational requirements.
  • Minimum of two years of relevant nursing experience.
  • Knowledge of home care regulations is essential.
  • Strong assessment, critical thinking, judgment, creativity, and decision-making skills.
  • Excellent verbal and written communication abilities.
