Job Overview:

This role provides direct patient care in a primary care office and works with care delivery providers to identify gaps in care. The Medical Home Medical Assistant coordinates messages through electronic portals, assists in managing transitions of care, and acts as a clinical liaison to the physician care plan.

The Medical Assistant participates in process improvements, is knowledgeable of clinical goals and outcomes, including patient satisfaction and engagement. This role requires strong skills in clinical care, customer service, communication, and teamwork.

Responsibilities:
  • Demonstrate proficiency in the rooming process for adult and/or pediatric primary care, including clinical procedures, immunizations, venipuncture, and point of care testing.
  • Follow protocols and policies for scheduling, clinical procedures, and appropriate use of medical equipment.
  • Provide accurate and complete documentation of clinical calls and patient rooming information, as well as order entry, pending prescriptions, and noting current pharmacy.
  • Address patient messages in a timely manner and escalate patient issues as appropriate.
  • Maintain knowledge of population health and embrace the philosophies of wellness, prevention, and chronic disease management.
  • Participate in tactics to close gaps in care, including pre-visit planning, daily huddles, and informing physicians of potential barriers to care.
  • Have a proficient understanding of the adult and/or pediatric preventative medicine schedule, including age-based wellness checks, appropriate vitals and labs, immunizations to administer, screening tools, and patient education.
  • Identify and communicate with providers regarding patient wellness, chronic diseases, utilization, and care gaps.
  • Attend continued education training and education, such as Lunch and Learns and other opportunities.
  • Participate in the longitudinal care continuum of patients, collaborating with wrap-around services, such as behavioral health, complex care nursing, social workers, and community health workers.
Requirements:
  • Graduate of an approved technical, professional, or vocational program in Healthcare.
  • Healthcare clinical experience preferred in a physician practice or related field.
  • Must be certified or registered medical assistant.
  • Must maintain certification and registration status, Registered Medical Assistant (RMA), through AMT, NCCT, or NHA, or certified Medical Assistant (CAN) by AAMA, and Cardiopulmonary Resuscitation (CPR).
Working Conditions:

This role requires standing, walking, and sitting for extended periods. The ability to lift and move patients, as well as use medical equipment, is also required.
