Key Responsibilities:
- Leverage data analytics to address care gaps in clinical quality metrics.
- Employ a patient-centered methodology and motivational interviewing techniques to enhance health outcomes.
- Assist with administrative functions within the quality department, including generating reports, entering survey data, and preparing for meetings.
- Manage population data using various computer applications.
- Guide patients and families in accessing, navigating, and connecting with internal services and external community resources. Provide education and outreach as necessary to enhance patients' quality of life and overall well-being.
- Support outreach initiatives and yearly plans to engage both current and new patients with services offered at the health center and with other specialty providers.
- Maintain registries and reports related to outreach efforts to bolster the outreach framework and strategy.
- Accurately schedule appointments in compliance with Neighborhood policies.
- Track and report metrics and outcomes associated with outreach initiatives.
- Demonstrate independent problem-solving skills to assist patients and providers in achieving care plan objectives.
- Advocate for patients when liaising with community agencies and services.
- Consult with healthcare providers regarding patient needs and facilitate appropriate referrals.
- Assist patients in obtaining and completing referrals as directed by providers, including securing prior authorizations when necessary. Document and monitor referrals until completion.
- Help patients access health insurance by connecting them with qualified enrollment facilitators.
- Collaborate as a team member with nursing staff, providers, community health workers, billing personnel, and other Neighborhood staff to ensure comprehensive patient care.
- Participate in training sessions and required meetings to foster professional growth and development through educational programs.
- Contribute to the development of procedures and policies related to responsibilities.
- Assist in creating and implementing metrics to evaluate the effectiveness of this role.
- Ensure thorough and accurate documentation of projects and assignments.
- Perform additional duties as assigned.
Qualifications:
- High School Diploma or equivalent required; Associate's degree preferred.
- Experience in Community Health, Patient Resource Coordination, or a related field.
- Proficient computer skills, clerical abilities, data entry, and organizational skills are essential.
- Exceptional written and verbal communication skills are required.
- Must be proficient in reading, writing, and speaking English; proficiency in Spanish is preferred.
