Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Summerdale, Pennsylvania.

Job Summary:

We are looking for a dedicated and compassionate Physical Therapist to provide high-quality care to our clients in their homes. As a Physical Therapist with BAYADA, you will have the opportunity to work with a diverse range of clients, from infants to geriatric patients, and develop a personalized care plan to meet their unique needs.

Responsibilities:
  • Conduct home visits to assess clients' physical abilities and develop a comprehensive care plan.
  • Perform diagnostic tests and measurements to identify areas of improvement.
  • Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
  • Continuously assess and revise the care plan to ensure optimal outcomes.
  • Educate and instruct clients, family members, or caregivers on rehabilitative care and activities necessary for independent living.
Qualifications:
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
