Job Title: Physical Therapist - Home Health

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Summerdale, Pennsylvania.

About the Role:

We are looking for a dedicated and compassionate Physical Therapist to provide high-quality care to our clients in the comfort of their own homes. As a Physical Therapist with BAYADA, you will have the opportunity to work with a diverse range of clients, from adults to geriatrics, and develop personalized care plans to meet their unique needs.

Responsibilities:
  • Conduct thorough assessments and develop individualized care plans to address clients' physical therapy needs.
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to promote clients' independence and mobility.
  • Collaborate with nursing staff and other healthcare professionals to ensure seamless care coordination.
  • Document observations, interventions, and evaluations accurately and efficiently using our state-of-the-art touch pad tablet.
Requirements:
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
