Job Title: Physical Therapist - Home Health Care

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in providing high-quality care to our clients in the comfort of their own homes.

About the Role:
  • As a Physical Therapist, you will work closely with our multidisciplinary team to develop and implement individualized care plans for our clients.
  • You will conduct diagnostic tests and measurements, develop and implement therapeutic exercises, and educate clients and their families on rehabilitative care and activities.
  • Our Physical Therapists work with adult and geriatric clients, providing a range of services including manual therapeutic exercises, gait training, balance, and other interventions.
Responsibilities:
  • Perform diagnostic tests and measurements to assess client needs.
  • Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
  • Continually assess and revise the care plan, participating with nursing in the multidisciplinary care plan as appropriate.
  • Educate and instruct clients, family members, or other client representatives on rehabilitative care and activities.
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
Requirements:
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
