Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled and compassionate Home Health Physical Therapist to join our team in Summerdale, Pennsylvania.

About the Role:

As a Home Health Physical Therapist, you will be responsible for providing high-quality, patient-centered care to adults and geriatric clients in their homes. You will work independently to assess and develop individualized care plans, conduct diagnostic tests and measurements, and educate clients and their families on rehabilitative care and activities.

Responsibilities:
  • Conduct home visits to assess clients' physical needs and develop personalized care plans
  • Perform diagnostic tests and measurements, such as mobility and strength assessments
  • Develop and implement manual therapeutic exercises, gait training, and other interventions
  • Educate clients and their families on rehabilitative care and activities
  • Accurately document observations, interventions, and evaluations
Qualifications:
  • Current Pennsylvania Physical Therapist license
  • Minimum of one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association
  • Ability to work independently and manage time effectively
  • Strong interpersonal, organizational, and problem-solving skills
