Job Summary

The Institute for Human Services, Inc. is seeking a skilled Program Referral Specialist to provide administrative referral support and facilitate communication with internal and external partners. This role ensures successful placements or resolutions of hospital and outside referrals to IHS specialty shelters, while monitoring data and reports for review by the Director of Operations.

Key Responsibilities:

  • Establish and maintain contacts with providers to promote program articulation and referral processes.
  • Function as a primary point of contact with hospitals to ensure timely and appropriate execution of referral correspondence, agency forms, and processes.
  • Implement IHS administrative procedures and practices for the program, assisting in the preparation and documentation of weekly provider meetings.
  • Prepare and draft agendas for various provider meetings and clinical/operational meetings.
  • Communicate with the Referral Coordinator and across teams to resolve hospital referrals and dispositions.
  • Maintain confidentiality and accurately record attendance, safety, and program data through SAMI and/or other systems as required.

Requirements:

  • Working knowledge of care coordination and healthcare principles.
  • Effective prioritization and problem-solving skills, with the ability to handle challenging behaviors.
  • Strong empathy skills to bridge IHS operations and referring hospital care teams.
  • Proficiency in Microsoft Office, including Excel, and database applications.
  • Possess personal qualities of integrity and commitment to the organization's mission.
  • Flexible and able to multitask, with the ability to work in an ambiguous, fast-moving environment.

Education and Experience:

  • High School diploma or GED equivalent.
  • At least one year of office experience in a fast-paced service environment.
  • Basic understanding of issues affecting homelessness and healthcare coordination.
