- Case Management: Develop individualized case plans focusing on housing stability, financial resources, and self-determination.
- Resource Connection: Assist participants in accessing community resources and provide advocacy as needed.
- Documentation: Maintain detailed case files, including assessments, progress notes, and discharge plans.

Qualifications:
