Key Responsibilities:

  • Outreach and Engagement: Conduct community outreach to identify eligibility criteria and engage with the target population.
  • Screening and Assessment: Perform screening interviews and assessments to determine program eligibility and needs.
  • Case Management: Develop individualized case plans focusing on housing stability, financial resources, and self-determination.
  • Resource Connection: Assist participants in accessing community resources and provide advocacy as needed.
  • Documentation: Maintain detailed case files, including assessments, progress notes, and discharge plans.

Qualifications:
