Key Responsibilities:

  • Coordinate care and services for select patients, including pre- and post-acute care.
  • Intervene with patients who have complex psychosocial and discharge planning needs.
  • Conduct detailed psychosocial assessments and coordinate with community resources and supportive counseling.
  • Offer crisis interventions to patients and families with psychosocial needs and facilitate the transition of care for high-risk patients facing barriers to care.
  • Manage all aspects of discharge planning for assigned patients to ensure timely discharge and connection with post-discharge care providers.

Requirements:

  • Master's degree in social work (MSW) or Master of Science in Social Administration (MSS).
  • Current state licensure as a Social Worker (LSW, LISW or LISW-S).
  • Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • Flexible, open-minded, and adaptable to change.

Preferred Qualifications:

  • Certification as a Case Manager within one year of eligibility from an approved professional organization.
  • Three years of experience in a healthcare system, including exposure to and experience in pre- and post-acute care.
  • Hospital social work experience.
  • Homecare and/or hospice social work experience.
  • Knowledge of community services.

Physical Requirements:

  • Ability to perform work in a stationary position for extended periods.
  • Ability to communicate and exchange accurate information.
  • Ability to work with physical records, such as retrieving and filing.
  • Ability to operate a computer and other office equipment.
  • Ability to travel throughout the hospital system.

Personal Protective Equipment:

  • Follows Standard Precautions using personal protective equipment as required for procedures.
