At Episcopal SeniorLife Communities, we are committed to providing high-quality services to individuals with disabilities and seniors. We are seeking a skilled Service Coordinator to join our team.

Job Summary:

The Service Coordinator will work collaboratively with individuals and their families to identify goals and locate support services that enhance their well-being. This role requires strong communication and coordination skills, as well as the ability to work effectively with a variety of stakeholders, including healthcare providers, insurance companies, and community support services.

Key Responsibilities:
  • Support and coordinate care needs with participants, their natural supports, and other waiver service providers.
  • Facilitate team meetings to ensure communication with the care team at least every 6 months.
  • Document in-person and telephone encounters in the case note and complete all follow-up activities.
  • Ensure participants maintain the level of care necessary to be eligible for the waiver (UAS minimum score of 5).
  • Facilitate referrals for indicated services, such as social services, housing assistance, vocational rehabilitation, and medical services.
  • Complete required documentation and meet all timelines and timeframes in accordance with program regulations.
  • Ensure participants maintain active Medicaid at all times and assist with benefit management for other benefits.
Requirements:
  • Bachelor's degree in a Health and Human Service field required.
  • Three years' experience providing care management for individuals with disabilities, mental illness, and/or seniors, and knowledge of community resources.
  • Valid New York State Driver's License required.
  • Flexible, yet organized, with the ability to exercise independent judgment.
  • Knowledgeable about community resources, Medicaid services, and available non-Medicaid services.
  • Demonstrated ability to collaborate effectively in a team setting.
  • Strong communication skills and ability to engage patients in a therapeutic relationship.
  • Ability to work with patients by telephone or in person.
  • Working knowledge of differential diagnosis of common mental health and/or substance use disorders.

Performing duties of this job requires occasional walking and standing in and around the reception area, and lobby.

Must be able to occasionally lift loads of 30 pounds without assistance and the ability to sit, talk, and hear is required.

Specific vision abilities required by this job include close vision, distance vision, peripheral vision, color vision, and the ability to adjust focus.
