Position Summary: The Clinical Supervisor plays a pivotal role in ensuring that patient care is effectively coordinated and managed. This position is accountable for the appropriate delivery of care and services, as well as the oversight of clinical staff.

Key Responsibilities:

  1. Handles case referrals and reviews pertinent patient information to assess hospice and home care requirements. Assigns suitable hospice staff to cases as necessary and collaborates with the Medical Director regarding eligibility queries.
  2. Evaluates each case through various methods, including home visits and record reviews, providing guidance to clinicians to enhance performance and ensure high-quality home care services.
  3. Reviews patients' medical histories, diagnoses, treatment plans, and clinical progress.
  4. Assists clinicians in setting immediate and long-term care objectives and developing comprehensive care plans.
  5. Participates in case conference meetings with hospice staff to ensure coordinated care and interdisciplinary collaboration.
  6. Conducts quarterly reviews of patient records and communicates findings to the Clinical Director and hospice staff.
  7. Supports the recruitment and onboarding process for new employees, offering recommendations for hiring.
  8. Collaborates with the Clinical Director and supervisory staff in planning and evaluating training and continuing education initiatives.
  9. Adheres to established professional standards and practices.
  10. Assesses community health needs and contributes to the development of programs to address these needs, while promoting the organization’s role in the healthcare system.
  11. Actively engages in quality assessment and performance improvement initiatives.
  12. Performs additional duties as assigned by the Clinical Director.

Qualifications:

  1. Must be a Registered Nurse with current licensure in the state.
  2. Preferred candidates will have a BSN from an NLN accredited nursing program or at least one year of nursing experience within the last three years.
  3. Experience in hospice or home care settings is essential, along with a minimum of two years in a management or supervisory role.
  4. Must comply with accepted professional standards and practices.
  5. Excellent observational skills, nursing judgment, and communication abilities are required.
  6. Familiarity with hospice philosophy and issues surrounding end-of-life care is important.
  7. Knowledge of pain and symptom management principles is necessary.
  8. Must possess a valid driver's license and have access to a reliable, insured vehicle.
  9. Current CPR certification is required.
  10. Demonstrated strong verbal and written communication skills are essential.
