The Nursing Clinical Supervisor is accountable for the effective coordination and management of patient care. This role acts as the primary contact for field clinicians, ensuring that support is readily available and timely.
Key Responsibilities:
This leadership role encompasses the oversight of clinical staff delivering care to patients in their residences. The position requires a blend of fieldwork and office presence, with specific responsibilities including:
- Providing daily oversight of patient activities, responding to incoming inquiries promptly, and ensuring necessary follow-ups to meet both internal and external customer requirements.
- Organizing training and educational initiatives for nursing staff in the field to enhance clinical outcomes and competencies.
- Evaluating staff learning needs and developing tailored plans and objectives for the upcoming evaluation period, while also performing administrative duties and serving as a resource for the team.
- Acting as the initial point of contact for field clinicians, assisting with patient care escalations and related matters.
- Facilitating bi-weekly case conferences with the clinical team to review patient progress and adjust care plans as necessary to achieve desired outcomes.
- Conducting thorough reviews of each case by assessing services rendered by clinicians, participating in conferences, reviewing records, and guiding clinicians to enhance performance and ensure quality home care services.
- Reviewing patients' clinical diagnoses, medications, procedures, and overall clinical progress.
- Assisting clinicians in setting immediate and long-term therapeutic objectives, prioritizing tasks, and developing comprehensive care plans.

Qualifications:
To excel in this role, candidates should possess:
- A minimum of two years of management or supervisory experience in a healthcare environment, preferably in home care or a primary care setting.
- A degree from an accredited nursing program; a Bachelor's degree is preferred.
- A current Registered Nurse (RN) license in California.
- A valid Basic Life Support (BLS) certification.
- A valid California driver's license with a satisfactory driving record.
