Job Summary

We are seeking a highly skilled Patient Care Coordinator to join our team at the Georgia Department of Public Health. As a key member of our public health team, you will play a vital role in supporting the overall mission and vision of protecting the lives of Georgian residents.

Key Responsibilities
  • Act as a client advocate to coordinate required services and resolve crises.
  • Counsel clients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life.
  • Identify environmental impediments to client progress through interviews and review of patient records.
  • Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plans.
  • Organize support groups or counsel family members to assist them in understanding, dealing with, and supporting the client.
  • Perform clinical assessments of the behavioral health needs of consumers presenting for services.
  • Provide case management services related to identified treatment needs.
  • Provide training to direct care staff in implementing behavior modification techniques and treatment plans.
  • Refer clients or family members to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement, or education.
  • Receive additional training, as required, to gain full proficiency and experience in all areas.
Requirements
  • Bachelor's degree in a related field from an accredited college or university and two years of experience in social service delivery.
  • High School diploma/GED with 4 years of experience in social service delivery.
  • Excellent customer-focused interpersonal skills to interact in an effective manner with practitioners, the interdisciplinary healthcare team, community agencies, patients, and families with diverse opinions, values, and religious and cultural ideals.
  • Excellent verbal and written communication skills.
  • Bilingual in Spanish/English, required.
  • Excellent computer skills, experience with electronic medical records a plus.
  • Experience with CAREWare/HMIS a plus.
  • Ability to maintain HIPAA compliance, corporate compliance, and client confidentiality.
