Job Description:

We are seeking a dedicated Nurse Care Coordinator to join our team. The successful candidate will be responsible for collaborating with patients, families, and healthcare teams to provide comprehensive care coordination services.

Responsibilities:

  • Assessing patients' needs and developing individualized care plans
  • Collaborating with healthcare providers to ensure seamless transitions of care
  • Providing emotional support and education to patients and their families
  • Maintaining accurate and up-to-date records of patient interactions and progress
  • Participating in quality improvement initiatives to enhance patient outcomes

Required Skills and Qualifications:

  • Current licensure as a Registered Nurse (RN) in the State of New York
  • At least 1 year of experience in a healthcare setting
  • Bachelor's degree in Nursing (BSN) or related field preferred
  • Excellent communication and interpersonal skills
  • Able to work independently and as part of a team
