Nurse Care Coordinator Job Description

COLON RECTAL ASSOCIATES OF CEN is seeking a skilled Licensed Practical Nurse (LPN) to serve as a Nurse Care Coordinator. This role will play a vital part in our team's success by providing exceptional patient care and support.

Key Responsibilities:

  • Work under the direction of a Registered Nurse to provide triage support and schedule patient appointments.
  • Assist providers with procedures, dressing wounds, and equipment cleaning.
  • Collaborate with fellow team members to ensure seamless patient care.

Requirements:

  • Current LPN licensure in New York State.
  • Previous experience in general surgery, colon rectal, or gastrointestinal field preferred.
  • Valid driver's license.

Salary Range:$65,000 - $85,000 per year based on experience.
