About the Role

We are seeking a skilled and compassionate Caregiver Field Specialist to join our team at All Ways Caring HomeCare. As a key member of our care team, you will play a vital role in ensuring the highest level of care and support for our clients.

Key Responsibilities
  • Conduct Home Visits: Meet with clients and their families in their homes to assess their needs and develop personalized care plans.
  • Coordinate Care Services: Schedule and facilitate timely follow-up visits to ensure the provision of services by our Direct Support Professionals.
  • Document and Track Information: Maintain accurate and up-to-date records of client interactions, including contact information and service delivery.
  • Communicate Effectively: Maintain ongoing communication with clients, families, coordinators, and program directors to ensure seamless care delivery.
  • Support Coordinators: Assist coordinators as needed to ensure the smooth operation of our care services.
  • Report Incidents: Document and report any significant incidents or accidents to the appropriate authorities.
Requirements
  • Education: High school diploma or equivalent; two years of supervisory experience or one year of supervisory experience with one year of college.
  • License and Certification: Valid driver's license from the state of residence with a satisfactory driving record; valid vehicle registration and insurance.
  • Skills and Qualities: Excellent customer service and communication skills; ability to function well under pressure, make decisions quickly, and manage a large amount of information.
  • Experience: Direct care and supervisory experience working with disadvantaged populations; knowledge and skills in information systems.
