Job Title: Patient Care Coordinator

At the Georgia Department of Public Health, we are seeking a highly skilled Patient Care Coordinator to join our team. As a key member of our public health team, you will play a vital role in supporting the overall mission and vision of protecting the lives of Georgian residents.

Job Summary:

The Patient Care Coordinator will provide exceptional support to clients and families, advocating for their needs and connecting them with essential resources. This role requires a strong understanding of social service delivery, excellent communication skills, and the ability to work effectively in a dynamic team environment.

Key Responsibilities:
  • Act as a client advocate to coordinate required services and resolve crises
  • Counsel clients in individual and group sessions to help them overcome dependencies and adjust to life
  • Identify environmental impediments to client progress through interviews and review of patient records
  • Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plans
  • Organize support groups or counsel family members to assist them in understanding and supporting the client
  • Perform clinical assessments of the behavioral health needs of consumers presenting for services
  • Provide case management services related to identified treatment needs
  • Provide training to direct care staff in implementing behavior modification techniques and treatment plans
  • Refer clients or family members to community resources to assist in recovery from mental or physical illness
Requirements:
  • Bachelor's degree in a related field from an accredited college or university
  • Two years of experience in social service delivery
  • Excellent customer-focused interpersonal skills
  • Excellent verbal and written communication skills
  • Bilingual in Spanish/English
  • Excellent computer skills, experience with electronic medical records a plus
