Role Overview
The Clinical Case Manager is responsible for coordinating patient care and overseeing program evaluations to ensure families navigate effectively through the continuum of care, ultimately aiming for a safe return to their home or community. This role includes maintaining essential documentation and liaising with external case managers and payors for insurance re-certification. Additionally, the manager will assist in coordinating patient/family financial responsibilities with Patient Accounts and maintain comprehensive program evaluation records, including outcome measures. Continuous analysis of trends and quality improvement reporting is also a key responsibility.
Qualifications
- Minimum of 1 year of experience in case management within a hospital setting.
- Master's degree in a relevant field.
- Experience in a rehabilitation hospital.
- At least 3 years of case management experience in a hospital environment.
- Bachelor's degree in a related discipline.
- Current licensure as a Registered Nurse in Georgia or Multi-State through the Enhanced Nurse Licensure Compact, or certification as a Certified Rehabilitation Counselor (CRC), Certified Disability Management Specialist (CDMS), Certified Case Manager (CCM), or registration in social work, occupational therapy, physical therapy, physical therapy assistant, or speech-language pathology.
- Ability to successfully complete the Basic Windows Skill Assessment with a minimum score of 80% within the first month of employment.
- Successful completion of Epic training within the first month of employment.
- Facilitates ongoing assessment and oversight of patient treatment plans and care levels, collaborating with clinicians, physicians, service providers, and educational programs to ensure appropriate care.
- Assists in planning for discharge and ensuring suitable placement post-discharge.
- Communicates with external case managers, physicians, and payors regarding patient progress and prepares necessary documentation for insurance re-certification.
- Coordinates discharge planning to ensure a seamless transition to the appropriate care setting, connecting families with community resources and support systems.
- Secures approvals and orders necessary equipment for post-discharge care.
- Maintains a therapeutic relationship with patients and families, ensuring clear communication tailored to their understanding.
- Identifies the need for ancillary services and facilitates referrals as needed.
- Participates in maintaining a work environment that meets Joint Commission readiness standards.
