Nurse Care Coordinator

We are seeking a compassionate and skilled Registered Nurse to join our team as a Nurse Care Coordinator in the Post Anesthesia Care Unit (PACU) at SSM Health.
Job SummaryThe Nurse Care Coordinator will provide direct nursing care to patients, advocating for their needs and collaborating with healthcare teams to ensure optimal patient outcomes. The ideal candidate will have excellent communication skills, be able to work effectively in a fast-paced environment, and possess a strong commitment to delivering high-quality patient care.
Responsibilities and Requirements- Assess patient needs and develop individualized care plans to promote optimal recovery and comfort
- Collaborate with interdisciplinary teams to coordinate patient care and services
- Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions of care
- Monitor patient progress and adjust care plans as needed to achieve desired outcomes
- Provide education and support to patients and families on health promotion, disease prevention, and management
- Current licensure as a Registered Nurse in Illinois, Missouri, Oklahoma, or Wisconsin
- Basic Life Support certification from the American Heart Association
- Experience in medical-surgical nursing, with a focus on patient advocacy and care coordination
- Excellent communication and interpersonal skills, with the ability to work effectively in a diverse team environment

Salary and BenefitsThe estimated annual salary for this role is $65,000 - $85,000, depending on experience and qualifications. Additionally, we offer a range of benefits, including:
