Nurse Care Coordinator for HIV Patients

We are seeking a Registered Nurse to join our HIV Clinic team at University Hospital in Madison, WI. As a Nurse Care Coordinator, you will deliver high-quality, individualized care to patients with HIV, focusing on their needs and building lasting relationships.

About the Job

This is an excellent opportunity to make a meaningful difference in the lives of our patients. You will work closely with our interdisciplinary team to provide comprehensive care, including nurse visits, patient education, and administering injections.

Responsibilities
  • Provide direct patient care, using electronic health records and other clinical tools to answer patient messages, triage patients, and perform other computer-related tasks.
  • Work collaboratively with our healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Conduct home visits or provide phone support to ensure seamless transitions between hospital and community-based care.

Requirements
  • Graduate of school of nursing required.
  • Bachelor's Degree in Nursing (BSN) preferred.
  • 6 months of RN experience required.
  • Relevant RN experience preferred.
