Nurse Care Coordinator

Key Responsibilities:
- Perform comprehensive assessments on all patients within the appropriate timeframe
- Plan individualized patient care, identifying problems/needs, nursing diagnosis, standards of care/practice, interventions, and attainable outcomes
- Coordinate patient teaching, discharge, and post-discharge needs as necessary for the patient population
Requirements:
- Earned a nursing diploma or degree from a college or university
- Licensed in the State of Tennessee
- BLS certification within 90 days of hire
- Basic arrhythmia certification within 6 months of hire (for 3rd floor telemetry unit, float pool, and critical care)
- ACLS certification within 6 months of hire (for 3rd floor telemetry unit, float pool, and critical care)

Estimated Salary Range: $75,000 - $100,000 per year, based on experience and location.
