**Key Responsibilities:**
- Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
- Maintain accurate and detailed records of patient interactions and care provided.
**Requirements:**
- Current North Carolina license as a Licensed Vocational Nurse / Licensed Practical Nurse.
- AHA BLS certification.
- Additional certifications related to Skilled Nursing care.
- Minimum 2 years of recent experience in Skilled Nursing.
