  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
  • Maintain accurate and detailed records of patient interactions and care provided.

**Requirements:**

  • Current North Carolina license as a Licensed Vocational Nurse / Licensed Practical Nurse.
  • AHA BLS certification.
  • Additional certifications related to Skilled Nursing care.
  • Minimum 2 years of recent experience in Skilled Nursing.
