Job Title: Clinical Care Coordinator

UHS is seeking a skilled Clinical Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for managing resource utilization and clinical outcomes for patients, ensuring seamless discharge processes, and collaborating with healthcare professionals to achieve optimal patient care.

Responsibilities:
  • Develop and implement effective care plans to meet individual patient needs
  • Collaborate with healthcare teams to ensure timely and efficient discharge processes
  • Conduct thorough assessments to identify patient needs and develop targeted interventions
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions
  • Participate in quality improvement initiatives to enhance patient outcomes and care delivery
Qualifications:
  • 2 years of previous acute care RN experience required
  • Bilingual preferred (Spanish)
  • Experience in Healthcare preferred
  • Must demonstrate commitment to UHS's Compliance Program and Code of Conduct
Education/Licensure:
  • Licensed in the State of Texas
  • Must have and maintain current BLS certification within 6 months of hire
  • Registered Nurse currently licensed in the State of Texas or part of a compact state license
  • BSN preferred
