Job Title: Clinical Nurse Liaison

 

About the Role:

We are seeking a highly skilled Clinical Nurse Liaison to join our team at Community Healthcare of Texas. This position offers an exciting opportunity to work collaboratively with hospitals and families to provide exceptional hospice care.

Key Responsibilities:

  • Referral Coordination: Evaluating patient eligibility for hospice services and responding promptly to referral sources.
  • Patient Discharge Facilitation: Completing necessary paperwork and collaborating with hospital staff, DME, medications, hospice physicians, and hospital staff to ensure seamless patient discharge.
  • Hospital Relations: Educating hospital staff, following up on pending patients, and building relationships through daily rounds.
  • Documentation and Reporting: Maintaining accurate and thorough documentation of hospice eligibility within required timeframes.
  • Strategic Partnership Development: Fostering strong relationships with hospitals to increase referrals and establish ourselves as the preferred hospice provider.

Requirements and Qualifications:

  • Licensure and Education: Registered RN in the State of Texas or LMSW required; nursing experience or medical training essential.
  • Marketing Experience: Proven marketing experience required; 1-3 years of healthcare marketing experience preferred.
  • Hospice Experience: Strongly preferred.
  • Technical Skills: Proficient computer skills required.
