Job SummaryWe're seeking a skilled Healthcare Coordinator RN/Transitions Expert to join our team in Fort Worth, TX. As a Field-Based HSS Clin Coordinator RN/Transitions, you will play a crucial role in assessing, planning, and implementing care strategies that are individualized by patient and directed toward the most appropriate level of care.
Responsibilities- Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate level of care
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
- Manage the care plan throughout the continuum of care as a single point of contact
- Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
To be successful in this role, you will need:
- A current unrestricted RN license in the state of Texas
- 2+ years of experience working within the community health setting or in a healthcare role
- Familiarity with Microsoft Office, including Word, Excel, and Outlook
- Reliable transportation and the ability to travel in assigned region to visit Medicaid members in their homes and/or other settings
We offer a competitive salary range of $65,000 - $85,000 per year, based on experience, as well as benefits such as reliable transportation reimbursement, flexible scheduling, and opportunities for professional growth and development.
