Job Title: Transition Services Specialist

Stamford Public Schools is seeking a highly motivated and dedicated Transition Services Specialist to join our team. As a key member of our special education department, you will play a vital role in supporting students with disabilities to achieve their post-secondary goals.

Job Summary:

The Transition Services Specialist will work closely with students, teachers, and families to develop and implement individualized transition plans. This will involve providing vocational and independent living skills training, as well as supporting students in their transition to post-secondary education and employment.

Key Responsibilities:
  • Develop and implement individualized transition plans for students with disabilities
  • Provide vocational and independent living skills training to students
  • Support students in their transition to post-secondary education and employment
  • Collaborate with teachers, families, and community agencies to ensure a comprehensive transition plan
  • Monitor student progress and make adjustments to transition plans as needed
Requirements:
  • Bachelor's degree in Special Education or related field
  • Experience working with students with disabilities
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Knowledge of transition services and vocational rehabilitation
