Transition Program Manager

We are seeking a highly skilled Transition Program Manager to join our team at the SEEM Collaborative. The successful candidate will be responsible for overseeing the transition planning process for students with exceptional needs, ensuring seamless transitions to adult services and supports.
Key Responsibilities- Review Transition Planning Forms and collaborate with the IEP Team to create IEP objectives aligned with students' post-secondary goals.
- Coordinate the school's community work program and develop employment opportunities for students in the community.
- Supervise job coaching staff, including one lead job coach and other job coaches.
- Provide consultation to classroom teachers to ensure community trips are centered on students' IEP and TPF goals.
- Plan and maintain a transition curriculum to be implemented through the classrooms with the support of classroom teachers and instructional aides/job coaches.
- Perform vocational and transition assessments for specific students to inform the IEP and team process.
- Coordinate with districts, state agencies, and local service providers in regards to the status of Chapter 688 referrals.
- Work with school administration to collect Indicator 14 data on post-school outcomes.
- Meet regularly with the Director of Transition Services to provide updates, gather feedback, and establish departmental priorities.
- Participate in team meetings, departmental meetings, and trainings as needed.
- Master's degree in education or a related vocational/transition field.
- DESE licensure in education or a similar certification in a related vocational/transition field.
- A valid Massachusetts driver's license and safe driving record.
- Preferred: Experience working with high school and transition age young adults with exceptional needs, DESE Transition Specialist Endorsement.
The Transition Program Manager will be evaluated on an ongoing basis by the Program Principal and will receive at least one formal evaluation annually by the Program Principal.
