Family Support Coordinator

About the Role:
At Communitas, Inc., we are dedicated to delivering individualized, family-centered services, programs, and support for people with intellectual and developmental disabilities. As a Family Support Coordinator, you will play a vital role in empowering families and supporting independence.
Key Responsibilities:
- Assist families in accessing community supports and services
- Help families obtain state and federal entitlement benefits
- Collaborate with other agencies and organizations
- Supervise support staff working in family homes
Requirements:
- Bachelor's Degree in Social Work, Education, Human Services, or a related field
- Up to two years of family support case management experience preferred
- Valid Driver's license, reliable transportation, and acceptable driving record
