Care Transition Specialist

This role is responsible for delivering exceptional customer service and support within assigned facilities to ensure a high level of satisfaction and timely transitions of care. The Care Transition Specialist will collaborate closely with key contacts within strategically assigned facilities to ensure understanding of services and benefits to achieve organizational referral, admission, and census goals and increase utilization of services within assigned accounts.

Key Responsibilities:

  • Collaborate closely with key contacts within strategically assigned facilities to ensure understanding of services and benefits to achieve organizational referral, admission, and census goals and increase utilization of services within assigned accounts.
  • Engage in pre-discharge facility calls/visits to prospective patients at the assigned facilities' request upon receipt of the referral to determine appropriateness and deliver seamless care transitions for home health care and hospice business lines.
  • Develop and execute annual education plan in assigned facilities and with key stakeholders in partnership with marketing, sales, & clinical teams.
  • Manage all aspects of the organization's care transition program & discharge follow-up calls, as assigned. Partner closely with clinical staff to ensure hospice readiness when appropriate.

Requirements:

  • 2+ years of healthcare experience as an LPN or RN (RN preferred) within a home healthcare or hospice setting. Must have a current license.
  • 2+ years of sales & customer service experience preferred.
  • Must have adequate computer skills.
  • Must complete and pass a state required background study.
  • Able to physically perform the duties required by the position.
  • Required to be able to read, write and speak English well enough to communicate with staff, patients, families, and facilities.
