About the Role

We are seeking a skilled and compassionate Family Medicine Registered Nurse to join our team at Mass General Brigham Community Physicians in Concord, Massachusetts.

The successful candidate will provide high-quality patient care, working collaboratively with physicians, advanced practitioners, and other healthcare professionals to deliver patient-centered care.

This is a full-time opportunity, working 40 hours per week, with a mix of patient-facing and phone-based responsibilities.

Responsibilities
  • Triage clinical questions via phone calls and provide clinical advice and education to patients.
  • Provide appropriate care coordination for patients, including scheduling appointments and referrals to specialists.
  • Collaborate with healthcare team members to develop and implement individualized care plans.
  • Provide direct patient care, including assessments, planning, implementation, and evaluation of care plans.
Requirements
  • Graduate of an approved school of nursing.
  • Current Massachusetts RN licensure.
  • Ambulatory/Primary care experience preferred.
