Key Responsibilities:

  • Maintain compliance with all organizational and departmental policies and procedures.
  • Assist in the preparation of medical records for new patients, ensuring adherence to licensing and certification standards.
  • Organize admission logs, index, and code diagnoses accurately.
  • Input data for Minimum Data Set (MDS) and physician orders as directed.
  • Conduct regular audits of medical records and submit findings to relevant stakeholders.
  • Ensure the completeness and accuracy of charts, addressing any discrepancies by coordinating with nursing staff.
  • Keep a detailed log of authorized information accessed from medical records.
  • Manage the check-out and check-in process for medical records efficiently.
  • Extract necessary information from records for insurance providers and government agencies.
  • Index medical records according to established guidelines.
  • Send daily physician telephone orders, track responses, and report any outstanding cases to management.
  • Generate required reports for healthcare providers.
  • Regularly manage the thinning and filing of medical records, ensuring compliance with established policies.
  • Retrieve medical records upon request from authorized personnel.
  • Assist with patient transfers and discharges as needed.
  • Complete Medicare certifications and recertifications accurately.
  • Compile discharge records and follow up on necessary documentation.

Qualifications:

  • 1-2 years of experience in healthcare is preferred, with experience in a skilled nursing environment being advantageous.
  • A high school diploma or equivalent is required.
  • Familiarity with medical terminology is beneficial.
  • Availability to work flexible hours, including weekends and holidays, as needed.
  • Proficient in English, with the ability to read, write, and communicate effectively.
  • Understanding of nursing and medical practices, regulations, and long-term care guidelines.
  • Competency in Microsoft Office, Outlook, and internet applications is essential for job performance.
