Responsibilities
  • Assessment and Triage: Conduct professional assessments and triage to determine the most appropriate treatment environment for each patient and/or family.
  • Communication and Collaboration: Develop and maintain effective relationships with patients, families, and healthcare professionals to ensure seamless transitions and optimal care.
  • Documentation and Reporting: Accurately document patient information and provide regular updates to healthcare teams and stakeholders.
Requirements
  • Education: Master's degree in Psychology, Counseling, Social Work, or a closely related field, or RN with current and active License in the State of Illinois as a Registered Professional Nurse.
  • Experience: Preferred 2 years of experience in psychological and chemical dependency assessment.
