In-home medical care for patients who are homebound has been shown to enhance patient comfort, improve health outcomes, lower healthcare costs, and boost satisfaction among providers and care teams. Job Summary
The Licensed Practical Nurse plays a vital role in the planning, coordination, and delivery of care for hospice patients in alignment with their individualized care plans. This position provides direct patient care under the guidance of a registered nurse. Responsibilities encompass adhering to the care plan, administering treatments, and collaborating with interdisciplinary team members to achieve favorable care outcomes.
Key Responsibilities
- Delivers direct patient care as outlined in the State Nurse Practice Act, following a patient assessment and an approved care plan initiated by the registered nurse. Responsibilities may include:
- Focused nursing visits
- Telephonic triage
- Night/weekend backup coverage as shared among the nursing team when the primary nurse is unavailable
- Additional duties as assigned by Hospice Leadership
- Collaborates with hospice team members regarding patient care.
- Conducts focused nursing assessments, identifies signs and symptoms of end-stage medical conditions, and implements planned nursing and medical interventions as directed by the registered nurse.
- Ensures compliance with Medicare Conditions of Participation.
- Responsible for collecting and reviewing documentation in electronic medical records, ensuring accuracy, completeness, and adherence to regulations and certification standards.
- Recognizes signs and symptoms of common medical conditions and implements planned nursing and medical interventions as directed by the registered nurse.
- Administers medications and treatments as prescribed by the patient's clinician, evaluates patient responses, and communicates concerns to hospice providers and/or registered nurses.
- Contributes to the development, implementation, or modification of the individualized patient care plan under the supervision of the registered nurse.
- Provides health education and support to patients and caregivers regarding end-of-life care needs.
- Assists with hospice chart audits as requested.

Experience and Education Requirements
Required
- Graduate of an accredited LPN training program
- At least one (1) year of experience as a licensed practical nurse in home health, hospice, skilled care facilities, memory care units, or acute care settings (preferably medical/surgical)
- Current unencumbered LPN license in the State of Oregon
- Valid driver's license, acceptable driving record, and automobile liability coverage or access to an insured vehicle
- CPR certification at hire or within six months of starting the position

Schedule: This position may occasionally require more than eight hours per day and may involve on-call or after-hours duties.
