Care Partners - A Leader in End-of-Life Care

Care Partners is a renowned non-profit hospice organization dedicated to providing compassionate and high-quality care to patients and families in the Portland metro area. We are seeking a skilled Nurse Practitioner to join our team and contribute to our mission of honoring choice and dignity at end-of-life.

Key Responsibilities:
  • Provide comprehensive hospice care to patients and families, including palliative medical services and symptom management.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and effective communication.
  • Facilitate goals of care conversations and assess patient prognosis to inform care decisions.
  • Manage symptoms and provide palliative care consultations as needed.
  • Participate in training and onboarding clinical staff to improve hospice and palliative care quality.
Requirements:
  • Current unencumbered Oregon licensure as an RN and national certification as a Nurse Practitioner in family practice or adult nursing.
  • Master's degree in advanced nursing or equivalent and good standing with Medicare.
  • Current unencumbered DEA license to prescribe schedule II medications.
  • Minimum of one-year professional nursing experience, preferably in hospice, palliative care, home health, oncology, and/or medical surgical care.
  • Excellent critical thinking, patient assessment, and problem-solving skills, including experience discussing end-of-life goals and wishes with clients.
