Patient Care Coordinator

Fresenius Medical Care North America is seeking a skilled Patient Care Coordinator to join our team. As a key member of our hemodialysis care team, you will play a vital role in providing high-quality patient care and support.
In this role, you will work closely with patients, healthcare professionals, and other team members to ensure seamless care coordination. Your attention to detail, excellent communication skills, and compassion for patients will make you an asset to our team.

Key Responsibilities
- Coordinate patient care activities, including scheduling appointments, preparing treatment plans, and communicating with healthcare providers
- Collaborate with patients, families, and healthcare teams to ensure comprehensive care coordination
- Develop and implement individualized care plans that meet the unique needs of each patient
- Monitor patient progress, identify areas for improvement, and make data-driven decisions to enhance care quality
- Participate in quality improvement initiatives and contribute to a culture of excellence within the organization
Requirements
- Current RN licensure in the state of practice
- At least 9 months of experience as a Registered Nurse (RN)
- Experience in chronic or acute hemodialysis (preferred)
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations
- Ability to prioritize tasks, manage multiple projects simultaneously, and maintain accuracy and attention to detail in a fast-paced environment
- Willingness to learn and adapt to new technologies, procedures, and guidelines
