Mental Health Nurse Practitioner Community ACTT Field Specialist

We are seeking a highly skilled and compassionate Psychiatric Mental Health Nurse Practitioner to join our Assertive Community Treatment Team as a Community ACTT Field Specialist. Job Description:Overview
The Psychiatric Mental Health Nurse Practitioner is responsible for providing high-quality patient care, assessment, diagnosis, treatment, and prevention of mental health disorders in a community setting. As a member of our Assertive Community Treatment Team, you will work closely with a multidisciplinary team to provide comprehensive care and support to patients and their families.
Key Responsibilities:
- Conduct thorough assessments of patients' mental health needs and develop personalized treatment plans.
- Prescribe and manage medications, ensuring optimal patient outcomes and safety.
- Collaborate with the treatment team to coordinate care and ensure seamless transitions between services.
- Provide education and support to patients and their families on mental health conditions, treatment options, and self-care strategies.
- Participate in daily team meetings and contribute to treatment planning discussions.
- Develop and maintain effective working relationships with community-based providers, hospitals, and other healthcare organizations.
Requirements:
- Master's or Doctoral degree in Nursing with a focus on Psychiatric Mental Health.
- Licensure as an Advanced Practice Registered Nurse (APRN) in North Carolina.
- Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC).
- Minimum 2 years of experience as a PMHNP in a community setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively as part of a multidisciplinary team.
Work Environment:
This position requires regular travel to community-based settings, including homes, clinics, and other locations. You will have opportunities to work with diverse populations and engage in various community-based initiatives.
Salary Range:
$120,000 - $150,000 per annum, depending on experience and qualifications.
