Job Description

As a Licensed Practical Nurse (LPN) at Team Select Home Care, you will play a vital role in delivering high-quality home health care to our patients. Your responsibilities will include:

  • Conducting in-home patient assessments and developing comprehensive care plans
  • Administering medications and treatments as prescribed by healthcare professionals
  • Monitoring patient progress and adjusting care plans accordingly
  • Collaborating with our interdisciplinary team to ensure comprehensive patient care
  • Maintaining accurate and up-to-date patient records
Responsibilities

Our ideal candidate will possess:

  • An active LPN license in good standing
  • CPR certification (must be up to date)
  • Strong communication and interpersonal skills
  • The ability to work independently and as part of a collaborative team
