- Communicate Effectively: Communicate timely and effectively with physicians, patients, family members, and other healthcare professionals regarding patient care issues or needs.
- Document Patient Visits: Document patient visits thoroughly and completely, maintaining an accurate and current medication listing.
- Participate in Performance Improvement: Actively participate in the performance improvement plan, working to reduce patient hospitalizations and improve outcomes.
- Adhere to Agency Policies: Adhere to and participate in the agency's Episode Management process and all other duties as assigned.
- Current LPN Licensure: Current LPN licensure in the state of practice.
- Driver's License and Vehicle: Current Driver's License, vehicle insurance, and access to a dependable vehicle.
- CPR Certification: CPR Certification required.
