Position Title: Registered Nurse Health Service

Reports To: Program Supervisor and Medical Director

Key Responsibilities:

  • Prepare patients for consultations by measuring and documenting vital signs such as blood pressure, temperature, pulse, weight, and conducting additional procedures as necessary.
  • Collect, record, and manage patient data including medical history, reports, and examination outcomes.
  • Review and update patients' medical records for accuracy before their appointments.
  • Assist in routine medical evaluations by preparing patients, necessary equipment, and supplies, and providing support to the healthcare provider as required.
  • Facilitate the completion of routine diagnostic request forms.
  • Occasionally assist in lifting patients with support, and handle medical instruments while addressing patient needs.
  • Schedule patients for tests and appointments with external facilities or specialists.
  • Provide guidance to non-clinical personnel on clinical matters (e.g., phone triage).
  • Administer treatments, medications, vaccinations, and specialized medical care to patients, including children and infants, to treat or prevent illnesses.
  • Refer patients to medical specialists or other practitioners when necessary.
  • Adhere to the organization's Mission Statement, Policies, and Structure.
  • Perform additional job-related duties as assigned.
  • Supervise and support medical assistants.
  • Ensure compliance with meaningful use and PCMH requirements and facilitate staff training.
  • Participate in Infectious Control training.
  • Lead support for the medical director during Continuous Quality Improvement (CQI) monthly meetings.
  • Generate CQI monthly reports on quality measures for presentation to the board.
  • Assist the quality assurance director in reviewing patient charts and reports.
  • Contribute to the annual UDS report and ensure adherence to all quality measures.
  • Maintain logs, including refrigeration temperature logs, VFC orders, and consent forms.

Qualifications:

  • Current NYS Registered Nurse license.
  • Current Infection Control Certificate.
  • Current BCLS certification.
  • Minimum of 2 years of relevant work, internship, or volunteer experience.
  • Bilingual in Spanish.
  • Proficient in computer technology.
  • Exhibit strong interpersonal relationship skills.
  • Demonstrate effective written and verbal communication abilities.
  • Ability to engage effectively with diverse populations.
