Key Responsibilities:- Design, recommend, monitor, evaluate, and document patient-specific pharmacotherapy to optimize medication use and ensure safety.
- Provide comprehensive pharmacy services, including medication access resolution, patient education, and clinical monitoring for conditions requiring specialty pharmaceuticals.
- Collaborate with healthcare teams to provide layered learning opportunities and educational programs for technicians, interns, students, and residents.
- Pharmacist license in the primary state of practice (PA).
- Secondary pharmacist licensure in NJ preferred.
- Bachelor's degree in pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy.
- Successful completion of a PGY1 Pharmacy Residency.
- PGY2 Specialty Residency preferred or equivalent experience in a hospital pharmacy setting.
