Pediatric Telephone Triage Nurse

Nemours Children's Health is seeking an experienced pediatric telephone triage nurse to join the Kids Health On Call program, supporting our primary care practices in Delaware and Pennsylvania.
Job SummaryThis is a fully remote, casual (16 hours per week) position that exclusively works weekend day shift. The Triage Nurse role provides primary care patients telephone access to a highly skilled and trained registered nurse who accurately answers their questions and addresses their medical concerns.
Key Responsibilities- Provide first line response to patient questions and concerns with escalation to the primary care on-call physician when necessary.
- Ensure continuity of care by communicating the patient's condition and plan of care to other health team members.
- Accurately assess each patient's condition by actively listening to callers and asking probative questions.
- Identify emergent situations and effectively transfer life-threatening calls to appropriate agencies per protocol.
- Provide appropriate and courteous telephone advice, including plan of care and patient education, to callers according to approved protocols.
- Accurately document initial and ongoing telephone conversations with documentation being completed immediately after the call ends.
- Place outbound follow-up calls to caregivers as needed.
- Communicate information to physician on call as needed, either over the telephone or by routing the necessary information to the physician through EPIC.
- Active unrestricted license for residential state to start.
- Active DE, PA, and FL Registered Nurse Licenses must be obtained within 90 days of hire date.
- Current plan for obtaining a Bachelor of Science in Nursing degree if not already achieved.
- Computer knowledge and skills, including word processing, databases/software (Cisco), desktop publishing, spreadsheets, patient scheduling, and EPIC.
