We are seeking a skilled and compassionate End of Life Care Specialist to join our team at Georgia Hospice Care. As a Registered Nurse, you will play a vital role in providing exceptional care to patients and their families during a challenging time.
Key Responsibilities- Deliver high-quality patient care in accordance with established standards and best practices.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with interdisciplinary teams to ensure seamless care coordination.
- Current state license as a registered nurse.
- At least 2 years of experience in a clinical care setting (hospice experience preferred).
- Reliable transportation and ability to lift up to 25 lbs.
