Key Responsibilities:

  • Act as the primary liaison for individuals on the planning list and their families.
  • Deliver comprehensive information about waiver services and the entry process for families seeking intellectual and developmental disabilities (I/DD) services.
  • Provide ongoing telephonic assistance to families awaiting services, particularly during transitions.
  • Address inquiries from individuals and families on the Planning List, ensuring prompt resolution.
  • Manage interactions with families under stress by employing patience and empathetic communication.
  • Collaborate with the Regional Field Office team to connect individuals with external agencies for non-waiver services and support.
  • Conduct extensive needs assessments for service seekers and those on the planning list, including annual evaluations.
  • Relay any changes or concerns regarding status and needs to the Planning List Administration Supervisor.
  • Compile and present detailed reports on the needs of individuals awaiting services to the Division of I/DD leadership.
  • Maintain regular attendance at the Regional Field Office during business hours; this is not a field-based position.
  • Exhibit strong organizational skills with the ability to track and follow up on various responsibilities.
  • Offer additional support to the Planning List team as required.

Qualifications:

A Master's degree in a relevant field from an accredited institution OR a Bachelor's degree in a related field from an accredited institution along with two years of pertinent experience.

Preferred Qualifications:

  • Advanced proficiency in Microsoft Office Suite applications.
  • Experience or understanding of individuals with intellectual/developmental disabilities, Medicaid waivers, and DBHDD I/DD services.
  • Familiarity with the IDD Connects information management system.
  • Ability to work autonomously without daily oversight.
  • Bilingualism or fluency in Spanish is a plus.
