We are seeking a highly motivated and compassionate Community Foundations Case Manager to join our team at InterCommunity. As a key member of our staff, you will play a vital role in supporting individuals struggling with substance abuse and mental health issues.

Key Responsibilities
  • Develop and implement individualized treatment plans to promote abstinence and sobriety.
  • Conduct regular case management sessions, including education and training, to empower clients towards recovery.
  • Collaborate with community providers to ensure seamless support services for clients.
  • Participate in admission and discharge processes, ensuring a smooth transition for residents.
  • Provide transportation and ensure client and facility safety and security.
  • Conduct substance abuse screenings and follow up with appropriate discipline.
  • Model positive behavior and serve as a role model for the community.
  • Supervise resident meal preparation, clean up, and daily chores, as needed.
  • Maintain confidential progress notes and other documentation on client activity.
  • Respond promptly to emergencies and incidents, ensuring a safe and supportive environment.
Requirements
  • High School Diploma or equivalent, with a commitment to pursuing an Associate Degree.
  • Two years of experience working with substance abusers in a residential setting, preferred.
  • Ability to work with diverse populations and a familiarity with Twelve Step Programs.
  • Strong initiative, leadership, time management, decision-making, and communication skills.
  • Proficiency in Microsoft Word and keyboarding, with basic computer literacy.
  • Excellent organization skills and attention to detail.
