Job Summary

We are seeking a highly motivated and experienced Certified Community Health Worker to join our Early Care and Education team at the Community Renewal Team (CRT). As a key member of our team, you will play a vital role in supporting the health and well-being of our customers, providing outreach, engagement, education, coaching, and advocacy services to promote healthy outcomes.

Key Responsibilities:

  • Provide outreach and engagement services to customers in the community, identifying those in need of case management and linking them with available services.
  • Conduct assessments of customer needs and develop individualized service plans to address their unique requirements.
  • Facilitate referrals to clinics and community support services, ensuring seamless coordination and communication between providers and customers.
  • Document all encounters and interactions with customers, including activities, ISPs, and outcomes achieved.
  • Participate in case management team meetings and provide reports to referring agencies, including progress reports, discharge summaries, and aftercare plans.

Requirements:

  • Bachelor's degree from an accredited college or university, or equivalent experience.
  • Minimum two years of experience providing community-based direct client services to populations experiencing substance abuse, homelessness, mental illness, and/or low income.
  • CT Community Health Worker certification, or ability to obtain within six months of taking the position.
  • Strong understanding of the dynamics of homelessness and ability to offer strength-based case management.
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse populations and stakeholders.
