Reentry Case Manager

The Community Partners in Action is seeking a skilled Reentry Case Manager to support participants returning home from prison to Hartford and the surrounding areas with their successful reintegration back into their communities. The ideal candidate will provide strategic case management services and assist participants with accessing the community resources needed post release.
Key Responsibilities
Provide long-term case management services to participants for at least up to 12 months in the community.
Work in collaboration with the programs Reentry Navigators to assist each participant to increase their knowledge and understanding of behavioral patterns in order to promote change and help end the cycle of recidivism.
Conduct participant risk/needs assessments using motivational enhancement strategies and formulate participants Individual Service Plans (ISPs).
Make appropriate participant referrals based on individual needs and collaborate with participants parole or probation officers to ensure post release compliance.
Requirements
The successful candidate will have a Bachelor's degree, preferably in Criminal Justice or Social Services, and at least two years of experience providing case management services. Experience may be considered in lieu of education. The ideal candidate will have strong emotional intelligence and written and verbal communication skills, knowledge of community providers that offer services to the criminal justice population, and ability to interact with a multi-cultural population and crisis management skills. A valid drivers license and reliable transportation are required. Bi-lingual capacity (English/Spanish) is preferred. A candidate in recovery for at least five years and/or formerly incarcerated is desirable. If formerly incarcerated, the individual must have completed parole and/or probation and have no criminal justice involvement for at least three years.
