At Community Renewal Team, we are seeking a highly motivated and compassionate Case Manager - Certified Community Health Worker to join our team. As a key member of our Supportive Housing department, you will play a vital role in providing support and services to individuals and families in need.
Job Summary:The Case Manager - Certified Community Health Worker will provide a wide range of services, including outreach, engagement, education, coaching, informal counseling, social support, advocacy, care coordination, and basic screening and assessments of social determinants of health. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with diverse populations.
Key Responsibilities:- Identify and provide outreach and engagement services to customers in the community.
- Conduct initial interviews and assessments to determine program eligibility and develop individualized service plans.
- Facilitate referrals to clinics and community support services, and coordinate services between providers and customers.
- Provide support, advocacy, and mediation services to ensure customers' overall needs and referrals are being conveyed.
- Document all encounters and interactions with customers, including activities, ISPs, and outcomes achieved.
- Conduct monthly home visits and bi-weekly conversations with customers, documenting all visits and conversations in the customer's file.
- Maintain client records in accordance with documentation requirements and standards.
- Assist clients in utilizing community services, including scheduling appointments with social services agencies and assisting with the completion of applications for programs based on eligibility.
- Participate in case management team meetings and provide reports to referring agencies, including progress reports, discharge summaries, aftercare plans, and special reports.
- Bachelor's degree from an accredited college or university required; education may be substituted with experience on a year-by-year basis.
- Two years of experience providing direct client services to populations experiencing substance abuse, homelessness, and/or mental illness required.
- Must have a CT Community Health Worker certificate or be able to obtain the certificate within six months of taking the position.
- Knowledge of working with the chronically homeless population, Housing First, and Harm Reduction Intervention strategies, Motivational Interviewing techniques required.
- Ability to work with individuals from diverse racial/ethnic and economic backgrounds, and to offer strength-based case management.
- Ability to communicate effectively with customers, funders, and vendors, and to function independently in a community setting.
