Patient Care Coordinator II

Position Overview: The Patient Care Coordinator serves as the primary liaison between patients and our healthcare facilities. This role adheres to Madison Medical's operational standards, policies, and procedures, promoting our mission within the clinic environment. This full-time position is under the supervision of the Clinic Supervisor / Assistant Director of Clinic Operations, specifically within the Gastroenterology department.

Key Responsibilities:
  • Exhibits professionalism and outstanding customer service, effectively communicating and assisting patients both in person and via telephone.
  • Efficiently schedules patient appointments and manages the physician's calendar in accordance with their preferences, including making reminder calls.
  • Handles incoming calls, screens inquiries, and takes messages from patients, processing them as directed by the medical team.
  • Accurately documents patient information in their records and collaborates with the care team to gather any additional necessary documentation.
  • Confirms insurance details and ensures that registration and payer information is accurate and up-to-date.
  • Identifies and rectifies registration discrepancies, utilizing insurance websites to verify patient eligibility when necessary.
  • Obtains prior authorizations for radiology and/or medications through web-based applications or phone communication.
  • Assists patients in arranging follow-up tests, including radiology, laboratory work, or specialist consultations.
  • Prepares correspondence for physicians and patients as required.
  • Performs additional duties as assigned.
Essential Qualifications & Attributes:
  • High School Diploma or equivalent.
  • Proficient data entry skills and familiarity with computer applications.
  • Highly organized with a keen attention to detail in a dynamic work environment.
  • Strong communication skills, comfortable interacting with patients, colleagues, and physicians.
  • Proven ability to work effectively both independently and collaboratively within a team.
  • Experience handling confidential information responsibly.
